This is a wonderful opportunity for students and parents to learn about what to expect at OHMS, elective options for students, how to register, ask questions and get a tour of the school.
Immunization Express Clinic @ OHMS 2/6
In order to attend the 7th grade Utah State law requires students must provide official documentation of immunizations. If documentation is not provided students will not be permitted to attend school or receive their class schedule in August 2014.
An immunization Express Clinic will be available from 4 - 6 at OHMS on February 6th. Insurance will be billed and a co-pay will be collected OR if eligible participate in the Utah VCF Program which charged $10 per vaccination administration.
Insurances accepted: Atena, Altius, Blue Cross, DMBA, Educators Mutual, PEHP, SelectHealth, Summit Care, United Health Care, Medicade, and CHIP.
Q. How much will it cost?
A. CNS is able to bill the insurances listed. If you have an office co-pay, CNS will collect that co-pay at the time of service and then bill the balance to your insurance.
Q. What if I have an insurance that CNS can't bill?
A. You have a couple options. You may either go to your pediatrician where they can bill your insurance, or you may still attend the Immunization Express Clinic, pay out of pocket, and then submit your receipt to your insurance for reimbursement. If you have insurance that CNS cannot bill, the out of pocket cost will be full price.
Q. If I have insurance, but just want to pay the $10 VFC rate, why can't I do that?
A. The VFC program is a State-funded program which provides immunizations for children who are not covered by insurance, or who have CHIP or Medicaid. CNS is prohibited to use this vaccine for children who have insurance, as that constitutes insurance fraud.
Q. My child doesn't have any insurance. How much will I pay?
A. If your child is eligible for the VFC program, each immunization would cost $10.
Q. We have a really high deductible on my child's insurance, why can't I pay the $10?
A. Unfortunately, a high deductible does not qualify for eligibly in the Vaccine-For-Children (VFC) program. This program is available only for children who have no insurance, or who have CHIP or Medicad.
7th GRADE SCHOOL ENTRY REQUIREMENTS
All of the kindergarten shots (listed below), plus a TDAP booster. the CDC also recommends at this age the following immunizations: Meningococcal and HPV.
Kindergarten Entry Immunization Requirements
- 5 TDAP
- 4 Polio
- 2 MMR (Measles, Mumps & Rubella)
- 3 Hepatitis B
- 2 Hepatitis A
- 1 Varicella (Chickenpox) - history of disease is acceptable
